Contractors License Bond Application
Prior to submitting information to obtain a quote on a contractors license bond, we require that you read the following privacy agreement and agree to (1) the conditions contained within and (2) further authorize Business Insurance Associates, Inc. to run personal credit based upon the information you provide.
Business Insurance Associates respects your right to privacy. We want you to know about our procedures for protecting your privacy and your rights and responsibilities regarding information we receive about you. We want you to understand how we gather information about you, how we protect it, and how you can help ensure its accuracy. Because we do not share your information outside of permitted exceptions, there is no need for you to take any action under this notice. If we change our practices in the future, we will advise you and, if applicable, enable you to “opt-out” of certain sharing.
What kind of information is collected about you and where did we get it?
We get most of our information about you directly from you, such as your name, address, social security number, income level and certain other financial information, on insurance and bonding applications, financial statements, and other forms that you provide to us. We may contact you by telephone, mail, fax or e-mail for additional information. We also keep information about the types of products and services you purchase from us, as well as account balances and payment history. We may also collect personal information from outside sources, such as customer reporting agencies and health care providers. This may include loss information reports, motor vehicle reports, credit reports and medical information.
Depending on the nature of your transaction with us, you may be required to provide Business Insurance Associates, or affiliates, insurance companies or other related entities working on our behalf with information. This may include, for example, your name, address, birthdate, phone number, health information, e-mail address, the types and numbers of the policies you hold, mother’s maiden name, social security number, credit card or bank account information, drivers license number, accident/violation history, information about vehicle operators, mortgages, lien/lease holders, vehicle information, or past credit history.
What do we do with information about you?
Information about you will be kept in our insurance policy/bonding records. We will refer to and use that information for purposes related to procuring and servicing insurance policies and handling claims. Generally, personal information about you in our records will not be disclosed by us to any external organization without your prior authorization. However, we may, as permitted by law, share information about you contained in our files with certain persons or organizations such as:
Your insurance company, adjusters, appraisers, auditors, investigators and attorneys, persons or organizations who need the information to perform a business, professional or insurance function for us or your insurance company, other insurance companies, agents or consumer reporting agencies as information is needed in connection with any insurance application, policy or claim involving you, to a court, state insurance department or other government agency pursuant to a summons, court order, search warrant, subpoena, or as otherwise required by law or regulation. Except as permitted or required by law, unless we obtain your written consent, we will not use or share any personally identifiable information about you for any purpose other than servicing your insurance policy, claim or account, or in a manner as previously disclosed to you by us when we collected such information. When your nonpublic personal financial or health information is disclosed by us to third parties for certain purposes described above, we will require them to use your information only for its intended purpose.
How do we protect your information and who has access to it?
We restrict access to nonpublic personal information about you to those employees, underwriters, and others as permitted by law, who need to know that information in order to provide products or services to you. We maintain physical, electronic, and procedural safeguards that comply with federal regulations to guard your nonpublic personal information.
